Pathway · Everyday & family care
One place that knows all of you.
Most families in Abuja collect doctors. A clinic near the office, another near the house, somebody's recommendation for the children, and a different hospital when it is serious. Nobody holds the whole picture, and every visit starts again from nothing. A family health home is the opposite arrangement.
What being registered actually changes
When your family is on one record, the person seeing your child at 9pm can already see that she reacted badly to a drug in March, that her weight has dropped since the last visit, and what her genotype is. Nobody has to ask you to remember, and nobody has to guess.
It also means somebody is watching the slow things. Blood pressure that has been creeping up for two years is invisible if every reading is taken at a different clinic. On one record it is a line on a chart, and it gets treated before it becomes a stroke.
What we handle day to day
- Everyday illness: fevers, malaria, typhoid, coughs, diarrhoea, rashes, injuries.
- Long-term conditions: hypertension, diabetes, asthma, arthritis, sickle cell, thyroid.
- Children: growth monitoring, immunisation, school and travel health, the first weeks after birth.
- Women's and men's health, including screening and contraception.
- Mental health, seen as an ordinary medical problem and treated as one.
- Medical fitness certificates for work, school and travel.
- Emergencies, around the clock, with no appointment needed.
The recall list
Chronic conditions are lost between appointments, not during them. Everyone means to come back in three months. Life happens. So the recall list exists: if you are being managed for blood pressure, diabetes or asthma, we contact you when a review is due rather than waiting to see whether you remember.
Reminders come by WhatsApp. You can opt out at any time, and you will not be contacted about anything you did not ask us to follow.
When it needs more than a consulting room
Being a family doctor inside a multispecialty hospital means the referral is a walk down a corridor. Diagnostics and imaging are in the same building, surgery has its own pathway, and where a child needs paediatric intensive care or a heart needs interventional work, the escalation runs to Limi Children's Hospital or CardioCare on the same record.
What people ask before registering
Do I have to register, or can I just come?
You can just come. Registration is what makes the record follow you afterwards, so it is worth ten minutes on a day when nothing is wrong.
Will I see the same doctor every time?
Where the rota allows, yes, and you can ask for continuity when you book. Where it does not, the record does the remembering.
Can my parents be on it too?
Yes. Older relatives are often the ones who benefit most, because they are the ones on several medicines from several places. See caring for the elderly.
Does my HMO cover this?
Good HMOs and primary NHIS are accepted. Plans differ on what they include, so confirm yours with us before treatment.
How it works
Four steps, then it runs itself.
Register
Names, dates of birth, history, allergies and current medicines for everyone you want on the record.
A baseline for each adult
Blood pressure, weight, sugar and a conversation about what runs in the family. It is the reference every later reading is measured against.
Come as needed
Everyday illness, injuries, the children, the things you have been putting off. Emergencies need no appointment at any hour.
We follow the long conditions
Reviews for blood pressure, diabetes and asthma are scheduled rather than left to memory, and referrals inside the group carry the record with them.
